» Committee Change Request
Students: Use this form to make a change in your graduate committee. It is not uncommon for committee changes to occur due to a change in research interests or because faculty have left the university. Be sure to obtain ALL required initials and signatures before submitting this form. This indicates that all parties are aware of, and acknowledge the changes. This form is submitted to the Graduate College and should be submitted when changes are made.
» Admission to Doctoral Candidacy
All doctoral students must be admitted to candidacy at least six months prior to completing degree requirements. Use this form to report successful admission to candidacy. Upon completion, this form is submitted to the Graduate College.
» Final Semester Verification Form (International Students Only)
The office of International Students and Scholars (ISS) has developed a Final Semester Verification (FSV) form to assist international students in meeting SEVIS mandates. International students must pick up the form from ISS (076 Student Union) and file it within the first two weeks of their final semester. The same form is used to apply for Optional Practical Training (OPT). To help students complete this form the faculty member who serves as the graduate student committee chair is urged to:
- Meet with the student to check and verify that courses taken (see the student's transcript on SIS) are on the Plan of Study.
- Update (revise) the Plan of Study by making corrections and changes by hand and initialing those changes as needed.
- Attach the revised Plan of Study to the Final Semester Verification Form for submission to the Graduate College. Please be sure the changes meet Graduate College and departmental rules (i.e., maximum number of -000 ending courses, GPA, etc.) and initial any deviations to these rules indicating you support the deviation.
- Ensure that the student fully completes and signs all needed sections of the FSV form (especially the first page).
- Ensure that the student files for diploma application with the Registrar's office.
- Sign the form. PLEASE NOTE: Committee chair and department head signatures indicate concurrence that the student will meet all departmental requirements by the end of semester (Plan of Study course and degree requirements, minimum GPA, thesis defense and submission deadlines, etc.).
- Once all items are complete, the student will submit the form (and revised Plan of Study if necessary) to the Graduate College for the dean's signature. This takes at least 24 hours to verify final semester requirements.
- The student returns to the Graduate College, picks up the form and takes it to the ISS office in the Union.
IMPORTANT --- PLEASE NOTE --- IMPORTANT --- PLEASE NOTE
The Graduate College will ONLY sign the FSV form if the:
- Student has fully completed the form;
- Plan of Study fully matches the transcript (and vice versa);
- Plan of Study meets all departmental and Graduate College requirements;
- Student's GPA is a 3.0 or above;
- Defense date for the thesis/dissertation is scheduled for at least one week prior to the semester deadlines and the advisor will verify that the student is on track to complete the research requirements by that date.
- PLEASE NOTE: This is the final semester ONLY IF the student will meet the thesis/dissertation deadlines established by the Graduate College that will enable the student to graduate in this semester. If the student will miss the established deadlines and will not graduate this semester, then this is not their final semester.

» Final Semester Verification Form (Domestic Students Only)
Domestic graduate students entering what they intend to be their final semester can submit a verification of last semester. This form requires concurrence from their academic advisor, thesis/dissertation committee (if applicable), department head, Dean of the Graduate College, and Registrar. This verification form must be completed with all signatures and filed with the Graduate College, prior to the end of the 2nd week of classes in order to receive permission to enroll in less than a full course of study.
» Graduation Clearance for Graduate Students
This form should be completed by the deadline as indicated in the academic calendar for that semester. Students should follow instructions and complete the top half of the form. Students should then work with their advisors to complete the remainder of the form. The completed form is then submitted to the Graduate College.

» Request for Extension to Submit a Plan of Study
Students enrolled in Master's degree programs are required to submit a Plan of Study before the completion of their 17 th credit hour. Doctoral students are required to submit their Plans prior to completing their 28 th credit hour. Due to circumstances beyond their control, there are occasions when this is not possible. Use this form to request a one-semester extension to submit a Plan of Study.

Form used by UNDERGRADUATE students who desire graduate credit for coursework taken in their last 12 hours of courses (see regulation guidelines on the second page of the form)
